The Clock on U.S. Measles Elimination Runs Out in November
Summary

Ninety-three percent of 2026's cases are in people who are unvaccinated or whose vaccination status is unknown, the same pattern seen in 2025. That points to the underlying number that actually predicts outbreak size: vaccination coverage.
Why: the coverage floor eroded
Measles needs a population that's roughly 95% vaccinated to stop chains of transmission before they start. National kindergarten MMR coverage sat at 95.2% as recently as the 2019–20 school year — the last time it cleared that line. By 2024–25, per CDC SchoolVaxView data reported by KFF, it had fallen to 92.5%, with only ten states still at or above 95%. Idaho, the lowest, sits at 78.5%; Connecticut, the highest, at 98.2%.

What one outbreak costs — and who does the work
The most complete accounting of what a U.S. measles outbreak actually costs comes from a single well-documented case: a 72-case outbreak that spread through Clark County, Washington, from December 2018 to April 2019. scientists and Washington state health officials reconstructed every dollar spent containing it and published the result in the pediatric research journal Pediatrics: $3.4 million total, $47,479 per case. About $2.3 million of that — two-thirds — was the public-health response itself: case investigation, contact tracing, lab work, and communication, meaning staff hours. Roughly $1.0 million was productivity lost by patients and contacts under quarantine, and about $76,000 was direct medical care for the 72 patients. A separate literature review folded into the same study found a median cost of $32,805 per case across ten earlier U.S. outbreak studies that counted only the public-health response — in every version of the accounting, most of the bill is labor.
That's the workforce now reporting it's heading for the door. PH WINS, the national survey of the state and local government public health workforce run by the de Beaumont Foundation and the Association of State and Territorial Health Officials, surveyed nearly 5,500 epidemiologists between September 2024 and January 2025. Twenty-seven percent said they plan to leave their current agency within the year; 77% reported at least one symptom of burnout.

A follow-up "Pulse Check" the same foundation ran in 2026 — after this data brief was published — found the trend accelerating: 41% of respondents said their work unit's budget was worse than in 2024, and the share of employees aged 61 and older planning to retire within the year roughly doubled, from 11% to 23%. The workforce that does the case-by-case labor a measles outbreak requires is thinning at the same time outbreaks are getting bigger.

The United States has been certified free of endemic measles since 2000. That status is reviewed, not permanent: the Pan American Health Organization convenes its Regional Verification Commission each year to check whether any member country has let the same measles virus lineage circulate, uninterrupted, for twelve months. The clock on the current U.S. outbreak chain started January 20, 2025. PAHO's review — originally scheduled for April 2026, then postponed at the request of U.S. health officials for further genomic analysis — is now set for November 2026. The Americas region as a whole already lost its regional elimination status in November 2025, after Canada crossed the twelve-month mark first.
